MEMORIAL WALL APPLICATION

The Memorial Wall at the memorial site consists of two basic parts. In the middle of the wall, the names of all firefighters from Iowa who were killed or died in the line of duty are listed. The second part of the wall is for firefighters who did not die in the line of duty. The criteria for this section is that the firefighters must have been a member of an Iowa fire department for at least 10 years, or died while an active member of that fire department. The cost of having this name put on the Memorial Wall is $100.00 and must be paid for by the family or that fire dept.

Below is a link to the form that must be filled out and then signed by the fire chief for a name to be included on the wall that was not a line of duty death. If you have someone you would like included on this section of the wall, go to the Memorial Wall Application below, print out the application, fill it out, and then send it along with a check for $100.00 payable to the Iowa Firefighters Memorial to:
